    mr google said,

"Pro forma" (Latin "as a matter of form") invoice is a preliminary document used to declare the value of the trade. It can be issued before the transaction to facilitate customs and payment arrangements.
This document models the final commercial invoice, and may look just like the commercial invoice except for the Pro forma invoice heading, but can also omit minor details.

    Cheryl
just do a normal invoice, and label it "Pro-Forma"
it is normally used so goods can be paid for before supply.

    Thank you Peter and Ian.
I did look on the net and the results I read left my head spinning in paper jargon and gobbledygook!!
The amount of paperwork that businesses are being put under to produce…my husband works in a bodyshop and the amount of paperwork that they have to go through for insurance jobs is enough to dishearten any professional who just wants to get on with the job…..

I don’t show the vat workings out on my proforma’s – just the total amount then beside it put + vat @ 17.5%

Basically it can be designed in anyway but I put the following:
    Your co name & address
    Their co name & address
    Proforma Ref Number
    Description of order
    Price
    Delivery date

I design mine in corel then email as a pdf.
    Cheers John

    Sorry forgot to add:
Work will only commence when payment has been received in full

Just use the invoice here too and label pro forma, when its comes down to issuing the invoice later a simple name change is all it takes
